2010年11月25日16时55分,××煤矿发生一起特别重大瓦斯爆炸事故,造成26人死亡,43人受伤,直接经济损失2130万元。
一、矿井概况

二、事故原因
1.事故的直接原因:矿井通风系统不合理,通风设施不合格,矿井漏风严重,放炮后涌出的瓦斯和掘进作业点溢出的瓦斯致使瓦斯积聚,达到爆炸浓度界限;因煤电钻综合保护装置供电电缆绝缘损坏,造成芯线短路,产生火花,引起瓦斯爆炸。
2.事故的主要原因
一是该煤矿无视国家法律、法规,拒不执行停产指令,违规建设,非法生产。该煤矿二号井以包代管,特殊工种无证上岗;矿井无正规设计,采掘布置混乱;矿井漏风严重,串联通风,未安装瓦斯监测监控系统;井下机电设备失爆现象严重。
二是××镇煤炭分局未采取有效措施彻底制止××煤矿违规建设和非法生产。××县煤炭工业局明知××煤矿手续不全却违规向该矿下发开工通知,明知××煤矿属于违规建设、非法生产矿井却研究上报置换保留该矿,且在煤矿整顿关闭工作中弄虚作假。××县煤矿安全监督管理局对××煤矿违反停建指令多次打开密闭违规建设和非法生产的问题监管不力。××市煤炭工业局明知××煤矿属于违规建设、非法生产矿井,却研究同意置换保留该矿,对煤矿整顿关闭工作中存在弄虚作假的问题失察。
