病史病人女性,65岁,小学文化,农民。主诉:发作性胸闷、气短5年,持续心前区疼痛5小时。病人于5年前出现劳累后胸闷、气短,当地医院诊断为“冠心病,劳累型心绞痛”,经治疗(具体治疗及用药不详)后症状缓解出院。此后,每于劳累后经常出现上述症状,含服硝酸甘油可缓解,未再入院系统治疗。入院前4h,病人再次于劳累后突然出现心前区疼痛,向左肩及背部放射,含服硝酸甘油不缓解,大汗,伴恶心、呕吐,家人急送医院。病人已婚,育有二子,配偶于2年前因,“结肠癌”病故。儿子均身体健康,与长子生活在一起家庭关系融洽,经济状况一般,性格开朗,平素喜食肥肉,无吸烟饮酒史。病人及家属对所患疾病的有关知识了解较少。
身体评估体温37℃,脉搏96次/分,呼吸24次/分,血压100/65mmHg。身高 162cm,体重60kg。神清,恐惧,查体合作,大汗末梢湿冷,口唇轻度发绀,颈软,双肺呼吸音略粗,心界不大,心率96次/分,可闻及2-3次早搏/分,心尖部第一心音减弱,腹软,无压痛,肝脾未触及,双下肢无浮肿,生理反射存在,病理反射未引出。
实验室及其他检查血常规:红细胞3.40×109/L,白细胞11.2×109/L,中性粒细胞78. 4%。血生化检查:CK103U/L,AST40.0U/L,LDH210U/L,CK-MB28.7U/L,血糖7. 2mmol/L,甘油三酯2.44mmol/L,cTn18.78μg/L。心电图:窦性心律,偶发室性期前收缩。Ⅱ、Ⅲ、aVF导联ST段弓背向上抬高,T波高尖,R波减低,V1~V5导联ST段压低,T波倒置。超声心动图:主动脉硬化。

At the same time, in America, Mike ,a friendly German boy, was also forming his own ideas. “I suppose I should criticize American schools” he says , “It is far too easy for our level but I have to say that I like it a lot In Germany, we do nothing but study. Here we take part in many other activities. I think that maybe American schools are better than in training their citizens. There ought to be some middle ground between the two.”
